Serving the Yolo County region, CommuniCare Health Centers provide comprehensive, high-quality healthcare services to anyone in the community regardless of insurance status or ability to pay. Their services are delivered by a dedicated team of providers and support staff at their clinic sites in Woodland, Davis and West Sacramento, and through outreach programs. Their goal is to develop and maintain long-term, healing partnerships with each of their patients while ensuring the best possible health outcomes.

Even during the COVID-19 pandemic, CommuniCare Health Centers continue to serve new and existing clients in the region. They offer substance use disorder screenings, referral and linkage, and treatment services via tele-health, and some in-person services have been moved to larger spaces that enable them to adhere to social distancing recommendations.
